Glossary: What's An HMO?

HMO (Health Maintenance Organization) plans offer rich coverage at reasonable costs. Under an HMO plan, each employee or individual chooses a primary care physician (PCP) from the carrier's HMO provider directory, and all routine services are provided by this physician.

If the employee or individual requires a specialist, out patient procedure or hospitalization, they "must" receive a referral from their PCP. This insures that the patient stays within the network, since out-of-network claims are not covered.

Generally, employees and individuals like HMO plans because the benefit structure is usually very high, with comprehensive routine and well-care packages included. Also, there are no claim forms to submit, as all paperwork is done on a referral basis where providers bill the insurance company directly.
