Annual Open Enrollment Is in April For May 1st Effective Date
For HMO choose DHMO Network
Employees may elect Dental and/or Vision coverage. Group plans are largely discounted and most often have better benefits than individual policies. In addition, premiums are deducted pretax from pay, lowering gross pay and taxes.
First Commonwealth Dental is an HMO. You must choose and use an DHMO Dental Provider to receive benefits.
Guardian Dental PPO plan is a 'Passive PPO.' Benefits are the same in or out of network. Guardian reimburses dentists a MAC (Maximum Allowable Charge). Most dentists will accept this reimbursement. We recommend asking non network dental providers to verify what Guardian will pay before dental procedures. Non network dental offices may balance bill member if they charge over the MAC.
